IURES 2025: Local Anaesthesia Enables Safe Transperineal Prostate Biopsy Technique

At the IURES 2025 Congress, researchers from SBU Sincan Training and Research Hospital presented results from a single-centre clinical study evaluating transperineal prostate biopsy performed under local anaesthesia. The study involved 34 patients and aimed to assess the safety, comfort, and diagnostic efficiency of the transperineal approach compared with the traditional transrectal method. 

Background and Objectives 

Prostate cancer remains the second most common cancer among men and the fifth leading cause of cancer-related mortality globally. The disease is particularly associated with advancing age, family history, and certain ethnic risk factors. Diagnosis typically begins with PSA testing and digital rectal examination, with elevated PSA levels or abnormal findings prompting a prostate biopsy. While the transrectal route has long been the standard technique, it carries risks such as rectal bleeding and infection, leading to growing interest in the transperineal approach. 

Study Findings 

In this study, the median age of patients was 64 years. The median PSA value was 10.15 ng/mL, with an average prostate volume of 70.4 mL and PSA density of 0.138 ng/mL². Histopathological examination confirmed prostate adenocarcinoma in seven patients. Throughout the procedure, the mean pain score was 4.9, while the average duration of biopsy was approximately 25 minutes. The mean IPSS, a measure of urinary symptoms, was recorded as 13.3. No major complications were reported overall.

Clinical Significance 

The team concluded that transperineal prostate biopsy under local anaesthesia is a safe, tolerable, and effective diagnostic technique. The approach reduces complications by avoiding the rectal wall and allows easier access to anterior prostate regions. According to the researchers, these results indicate that transperineal biopsy represents a reliable alternative to the transrectal method and may enhance both patient comfort and diagnostic precision in clinical practice.
