Lasers Versus Microneedling in Post-Acne

Post-acne scarring remains a common dermatologic concern with meaningful effects on quality of life, and both fractional CO2 laser and microneedling radiofrequency are widely used in clinical practice. This meta-analysis pooled data from eight randomized controlled trials involving 249 patients to compare the efficacy and safety of these two procedural approaches in the management of post-acne scarring.

The analysis concluded that fractional CO2 was the superior method for treating acne scars and was associated with higher patient satisfaction scores, suggesting a better clinical response. Notably, fractional CO2 was also associated with significantly greater pain scores than microneedling radiofrequency.

Safety Tradeoffs Shape Treatment Decisions

That greater efficacy came with a clear tolerability cost. Fractional CO2 laser was associated with significantly higher pain scores, a markedly increased risk of post-inflammatory hyperpigmentation, and a longer duration of erythema. In contrast, microneedling radiofrequency showed a more favorable safety profile across these outcomes.

While fractional CO2 laser may offer stronger results for scar remodeling, microneedling radiofrequency may be the better fit for patients who prioritize comfort, reduced downtime, or lower risk of pigmentary complications. This may be especially relevant when counseling patients with differing skin types, lower pain tolerance, or limited ability to accommodate recovery time.

Individualized Care Remains Essential

The findings reinforce that treatment should not be driven solely by efficacy, but rather by an individualized approach based on each patient’s priorities and goals, including pain tolerance, expectation for recovery, and concerns regarding post-inflammatory hyperpigmentation.

The authors highlight the need for further research with longer follow-ups and more standardized outcome measures. For now, this meta-analysis suggests that fractional CO2 laser may be the stronger performer for efficacy, while microneedling radiofrequency offers a safer and more tolerable alternative.
