Midlife Vascular Risk Costs Up to 13 Dementia-Free Years

MIDLIFE vascular risk burden significantly reduces dementia-free survival years among middle-aged adults over decades of follow-up. In a prospective community-based cohort study analyzing 12,409 participants followed from age 55 to 95, researchers evaluated how cumulative cardiometabolic risk factors—specifically diabetes, hypertension, and current smoking—influence long-term cognitive longevity. Over a median follow-up of 26.3 years, 3,008 individuals developed dementia and 5,238 died without dementia. Evaluating the Midlife Vascular Risk Burden demonstrated a strong, dose-dependent relationship with reduced cognitive lifespan.

Impact of Midlife Vascular Risk Burden on Survival

Compared to individuals with no vascular risk factors at baseline, adults presenting with three risk factors exhibited a 2.69-fold increased hazard of dementia (hazard ratio [HR] 2.69; 95% CI: 1.94–3.73) and a 5.61-fold increased hazard of death without dementia (HR 5.61; 95% CI: 4.67–6.74). Using restricted mean survival time analysis, participants with zero risk factors averaged 30.1 dementia-free survival years between ages 55 and 95. In contrast, dementia-free survival decreased to 26.3 years for one risk factor, 21.0 years for two risk factors, and 17.5 years for three risk factors. Maintaining optimal vascular health in midlife was associated with up to 12.6 additional years lived free of dementia.

Clinical Implications and Demographic Variations

Although participants with higher vascular burden experienced earlier dementia onset, their lifetime cumulative incidence of dementia by age 95 appeared lower (23% vs 42% for those with zero risk factors) due to substantially higher competing mortality before reaching older ages. Subgroup analyses demonstrated that women lived longer dementia-free than men across all risk levels, whereas Black participants experienced shorter dementia-free survival than White participants. Additionally, apolipoprotein E (APOE)-ε4 carriers had reduced dementia-free survival compared with non-carriers.

Overall, addressing Midlife Vascular Risk Burden in clinical practice provides a pivotal opportunity for cognitive disease prevention. Translating cardiometabolic risk into time-based outcomes offers clinicians an effective tool to communicate risk, encourage lifestyle modifications, and preserve cognitive function alongside cardiovascular health.
