NHS HIV Testing Programme Discovers Nearly 2,000 Undiagnosed Cases

Nearly 2,000 people unknowingly living with HIV have been diagnosed through a pioneering NHS emergency department testing programme, according to new NHS England data. The opt-out blood testing initiative, introduced in April 2022, has identified 1,900 previously undiagnosed HIV cases while also detecting hepatitis B and hepatitis C infections in high-prevalence areas across England.

The programme offers routine testing to adults undergoing blood tests in emergency departments unless they actively choose to opt out. It is currently available in 88 locations across England with the highest rates of HIV and is being hailed as both clinically effective and cost-effective.

Programme Reaches Individuals Unlikely to Seek Testing

Data from the UK Health Security Agency (UKHSA) suggest the initiative is successfully reaching individuals who may not otherwise undergo HIV testing. Research found that 93% of people diagnosed through the programme had no previous record of an HIV test.

One in 20 people living with HIV in England remain unaware of their diagnosis, increasing the risk of delayed treatment and onward transmission. Early diagnosis enables prompt access to antiretroviral therapy, allowing individuals to live long, healthy lives while preventing transmission of the virus.

Patients who test positive through the programme are immediately referred for specialist NHS support and offered an individualised treatment plan.

Study Finds Testing Saves Lives and Prevents Transmission

A recent economic evaluation examined the programme’s first 802 HIV diagnoses and estimated that it will prevent approximately 187 HIV-related deaths and 28 new HIV transmissions over the next two decades.

Researchers from the National Institute for Health and Care Research (NIHR) Health Protection Research Unit at the University of Bristol concluded that the programme represents good value for money for the NHS. At approximately £6 per HIV test, the intervention remains cost-effective despite the large number of tests required to identify each undiagnosed case.

The study found that earlier diagnosis enables individuals to begin life-saving treatment sooner, improving long-term health outcomes while reducing healthcare costs associated with advanced HIV disease.

Supporting England’s HIV Elimination Goals

The programme forms part of England’s HIV Action Plan 2025–2030, which aims to end new HIV transmissions by 2030. To support this ambition, the government has committed £156 million between April 2026 and March 2029 to continue and expand emergency department testing for HIV, hepatitis B, and hepatitis C.

Three additional hospitals are expected to begin offering routine opt-out testing by April 2027, extending coverage to almost all areas of England with very high HIV prevalence.

Health leaders have highlighted the programme as an example of how routine healthcare interactions can be used to improve prevention and early diagnosis.

Earlier Diagnosis Improves Outcomes

The success of the initiative demonstrates the potential benefits of normalising HIV testing within routine healthcare settings. By identifying infections earlier, the NHS programme is helping to reduce stigma surrounding HIV testing while improving patient outcomes and supporting wider public health efforts.

With continued investment and expansion planned, routine opt-out testing is expected to play a key role in England’s strategy to eliminate new HIV transmissions over the coming decade.
