Secukinumab Shows Sustained Long-Term Effectiveness and Safety

NEW long-term real-world data from Italy provide further evidence supporting the durability and safety of secukinumab in moderate-to-severe plaque psoriasis, according to findings from the IL PSO multicentre retrospective study.

The study analysed 769 adult patients treated across 15 Italian dermatology units who remained on continuous secukinumab therapy for at least four years, with follow-up extending up to seven years. Researchers assessed long-term effectiveness using Psoriasis Area and Severity Index (PASI) responses, including PASI 90, PASI 100 (complete skin clearance), and absolute PASI ≤2, alongside safety outcomes.

Secukinumab Shows 7-Year Psoriasis Efficacy

Efficacy results demonstrated a progressive improvement in clinical response over time. PASI 90 was achieved in 36.0% of patients at Week 16, increasing to 71.0% at Year 1, 84.1% at Year 4, 87.0% at Year 5, and 90.0% at Year 6. Similarly, PASI 100 responses rose from 27.4% at Week 16 to 67.1% at Year 4 and reached 80.2% by Year 7, indicating sustained and even increasing rates of complete skin clearance with prolonged treatment.

Subgroup analysis suggested that patients with concomitant psoriatic arthritis experienced enhanced outcomes, with significantly higher PASI 100 rates observed at Years 5 and 6 compared with those without joint disease (p≤0.05). These findings may indicate potential benefits of IL-17A inhibition in patients with broader inflammatory disease burden.

From a safety perspective, secukinumab demonstrated a favourable long-term profile. The overall incidence of adverse events was low, and serious adverse events were infrequent. Treatment discontinuation due to adverse drug reactions occurred in five patients, including three cases of de novo malignancy, one case of eosinophilic pneumonia, and one pulmonary embolism. No new or unexpected safety signals were identified over the extended follow-up period.

Long-Term Treatment Considerations and Outlook

The authors conclude that in this selected cohort of long-term responders, secukinumab maintained durable efficacy with a progressively improving response profile over time, alongside a reassuring safety record up to seven years in real-world clinical practice.

These findings add important long-term evidence to the existing clinical trial data, reinforcing the role of secukinumab as a sustained treatment option for patients with chronic moderate-to-severe psoriasis, particularly in real-world settings where long-term persistence and effectiveness are critical considerations for dermatology practice.
